E-rate Update - Existing Contracts Information Needed
Message Posted February 11, 1998

PA OFFICE OF EDUCATIONAL TECHNOLOGY SURVEY
TO SCHOOLS AND LIBRARIES SUBMITTING FORM 470s FOR EXISTING CONTRACTS

As of Tuesday, Feb. 10, more than 800 Pennsylvania schools and libraries had applied to participate in the E-rate program -- more than any other state in the country! We are delighted that so many of you are applying to receive discounts, but are concerned that 800 only represents less than a fourth of the total number of eligible entities in PA.

In order for us to target our outreach efforts in the upcoming weeks, we need your help identifying which schools and libraries have submitted or will be submitting Form 470s for EXISTING services. Because, as many of you who have been on the SLC web site realize, it is possible to view the list of schools and libraries who have submitted Form 470s for NEW services; however we are NOT able to view a list of which schools and libraries submitted Form 470s on behalf of EXISTING contracts.

If you have applied or will be applying for discounts on EXISTING contracts, please take a minute and provide us with some valuable information, preferably by 5:00, Friday, February 13. You can either reply to this message, write to [00jtritt@psupen.psu.edu], or fax (717) 787-7222. Where applicable, answers may just listed as yes or no.

1) School or Library Name
2) County
3) AUN, if readily available
4 a) Have you submitted Form 470 for existing services? YES or NO
b) If no, WILL you be submitting Form 470 for existing services?
YES or NO
5) If the answer to either 4a or 4b is YES, please indicate for which
services.
Telecommunications services? YES or NO
Internal connections? YES or NO
Internet access? YES or NO
6) Have you submitted Form 470 for BOTH existing and new services?
YES or NO

Thanks in advance for your time. The results of this survey will be posted in a future message.
