BEAR FORM DEADLINE OF JANUARY 31, 1999 WILL NOT BE ENFORCED
The SLD is aware that some applicants may not be able to meet the January 31, 1999 deadline for filing their Billed Entity Applicant Reimbursement (BEAR)
Forms due to the timing of the receipt of their funding commitment decisions letters, and due to the processing time for completing the BEAR Form
(including obtaining a signed Block 4 Acknowledgment from their service provider).
Please be advised that the SLD will NOT ENFORCE the January 31, 1999 deadline for submitting the BEAR Form. Applicants are strongly encouraged to submit
their BEAR Forms as soon as possible after receiving their funding commitment decisions letters. Until we receive and process your BEAR Forms, your service
providers are not required to reimburse you for eligible services you have paid for to date, nor will they be able to begin rendering discounted bills
for your services. Once all of our funding commitment decisions letters for the first program year have been issued, SLD will announce a final deadline
for submission of all BEAR forms still outstanding.
