E-rate Update: Type-in/Print-out Forms Now Available

The SLD has just made available a tool that many Pennsylvania applicants have been asking for for more than a year -- type-in versions of Forms 470
and 471. You can now type-in and print out your forms rather than filling them out by hand or submitting them on line. They cannot then be submitted electronically, you then must mail them to the SLC. They can be found in the Reference Section of the SLD web site under 470 and 471 guidance. More specifically:

Two new application tools are now available from the Schools and Libraries Division of USAC: versions of the Form 470 and Form 471 that you can download
from the SLD Web Site, fill in on your computer, print out, and mail to us. They can be found in the What's New section of the SLD Web Site, www.slcfund.org . These type-in/print-out forms represent another alternative to filing your Forms 470 and 471 online or hand-writing/typing these applications. These Forms 470 and 471 are virtually identical to the PDF files that have been available on our Web Site, but now you can enter information directly into the forms rather than just printing our blank hard copies and filling them in by hand.
These forms do not electronically transmit data to the SLD, but instead make your completion of the paper forms easier and neater. You must have Adobe's
free Acrobat Reader 3.01 installed on your computer in order to access the Type-In/Print-Out Forms 470 and 471. See our Web Site on how to obtain this
software as well as specific instructions for downloading these forms. In addition, the SLD has posted Helpful Hints for using the Type-In/Print-Out
forms. They can be found in the Reference Area of the Web Site under 470 Application Guidance.
Lastly, those of you who may have experienced timing out and other errors while accessing our Web Site will be happy to know that we've made substantial
progress over the past three days in correcting these problems. Please do visit the interactive areas of the Web Site again soon, and let us know how
we're doing in improving site performance.
