Form 486 Correction

Revised E-rate Form 486 Must Be Used in Year 2
The Schools and Libraries Corporation has just informed us that they will be changing the Form 486 for Year 2, which explains why all of our funding recipients in Wave 1 are receiving their Funding Commitment Letters with no 486 enclosed. From what we know, the form will not be drastically changed, but will include two additional certifications. The data entry sections will remain about the same.
As many of you told me today, the letters for Wave 1 (in addition to Wave 2) contain erroneous language in them about "using the enclosed 486 and submitting it within 5 business days." For Waves 1 and 2 funding recipients, the revised 486 will be mailed within the next week. Then revised 486s will be enclosed in the letters beginning with Wave 3. In addition, the revised 486 will be on the SLD web site http://www.sl.universalservice.org within the next few days for you to download. Please do not use the old 486 for Year 2 funding commitments, as they will have to return it and ask you to submit a new, revised 486.
Finally, if you are still filing 486's for Year 1 (internal connections extensions or contract extensions for contracts ending before December 31) you should plan to use the new 486.
