Important Reminder: Year 4 Form 486's Must be Postmarked by Oct. 27
Message Posted October 17, 2001

Beginning with year 4, E-rate recipients must certify by Oct. 27
that they are CIPA compliant by mailing the Form 486. This is unlike other
years where you have had an almost endless amount of time to submit your
Form 486 (up until the time your BEAR was submitted).
If you do not have your 486 postmarked by Oct 27, the SLD will only
provide E-rate discounts from the date that the 486 is submitted. In other
words, if you wait until January 1 to submit your 486, you will lose your
E-rate discounts from July 1 - Jan. 1.
If you have not been funded in waves 1-6, don't worry. You will
have 120 days from the funding commitment letter date to submit the
associated 486. To be certain that you haven't been included in waves 1-6,
do a quick search on the SLD website at:
http://www.sl.universalservice.org/funding/
Important notes:
1) Because the 486 postmark is now crucial, please consider sending
your form using either an express delivery service or return receipt in
order to verify delivery.
2) If you are only receiving telecom services you are not required
to comply with CIPA, however you ARE required to comply with the Oct. 27 486
deadline.
3) Consortiums filing Form 486s for Internet access and/or internal
connection services are certifying that they have received Form 479 CIPA
certifications from all consortium members (even those receiving only
telecom services).
