E-Rate Wave 11 Funding Committments

Congratulations to the following 4 Pennsylvania schools and libraries that will be receiving Year 5 Funding Year 2002-2003), Wave 11 E-rate Funding Commitment Letters, totaling $29,939.92. A summary of each commitment is listed below, and detailed information will be posted on the SLD web site under Funding Commitments: http://www.sl.universalservice.org/funding/.
If you find that your funding commitment letter does not arrive in a timely manner, please call the SLD ASAP to get a replacement (888-203-8100). All Year 5 correspondence will be in BLUE.
Currently Schools and Libraries have funded approximately 80% of the funding requests received for Year 5 and funding is still only being provided for Priority One (Telecommunications and Internet Access) services at this time.
* The next step is to file the Form 486 which tells the SLD 3 things:
1) that they are authorized to pay invoices (either BEAR or SPIFs)
2) that your technology plan has been approved (and by which SLD-certified approver)
3) that you are or will be CIPA compliant by July 1, 2002.
If your school or library has not been funded in any of the Year 5 waves and your application does not include Internal Connections, you can check the status of your application by accessing the SLD's web site at http://www.sl.universalservice.org/menu.asp and clicking on Application Status under the Form 471. (Note this is the same area where you apply on-line) You will need to provide your Billed Entity number and the Funding Year you wish to review.
Also please keep the following important dates in mind:
Year 4 BEAR Forms -
All Year 4 BEAR Forms must be received and postmarked by the SLD no later than October 29, 2002. (120 days after the last service date - June 30, 2002)
Year 5 Form 486
Received or postmarked no later than 120 days after the date of the Funding Commitment Decision Letter or 120 days after the Service start Date, whichever is later.
Examples:
If you received your Year 5 Funding Committment Letter on May 5, 2002 and your Service Start Date is July 1, 2002, your Form 486 must be postmarked no later than October 30, 2002 (120 Days after July 1, 2002)
If you received your Year 5 Funding Committment Letter on July 15th, your Form 486 must be postmarked no later than November 12, 2002 (120 days after July 15, 2002).
