Year 6 (FY 2003-2004) Wave 8 E-rate Funding Committed

Congratulations to the following 9 Pennsylvania schools and libraries that will be receiving Year 6, Wave 8 E-rate Funding Commitment Letters, totaling $10,640,071.62.
A summary of each commitment is listed below, and detailed information will be posted on the SLD web site under Funding Commitments at: http://www.sl.universalservice.org/funding/. Your letters are being mailed on Monday. If you find that your funding commitment letter does not arrive in a timely manner, please call the SLD ASAP to get a replacement (888-203-8100). All Year 6 correspondence will be sent in YELLOW.
Again this week, the SLD is funding internal connections requests at the 90% level. This wave, therefore, consists of both priority 1 and 2 requests. There are many, many more waves coming (there were 30 waves in Year 5), and so if you haven't received your FCDL yet, don't panic. Your commitment likely will be in one of the upcoming waves.
If you do not agree with your funding commitment decision letter, you have 60 calendar days to appeal from the date listed at the top of your FCDL. Appeal instructions can be found at:

A few very important notes:
YOUR FCDL IS NOT THE LAST STEP. YOU NOW MUST FILE THE FORM 486.
* The 486 tells the SLD 3 things:
1) that they are authorized to pay invoices (either BEAR or SPIFs)
2) which SLD-certified approver approved your technology plan
3) that you were CIPA compliant by start of services
This form must be filed within 120 of the service start date or the date on the funding commitment letter, whichever is later. And since most services begin on July 1, you have until the end of October to have this form filed with the SLD. If you are receiving internal connections funding, you may want to wait until after the product is delivered or installed to complete the 486.
The 486 is available in paper form under the "Forms" section on the left side of their redesigned website, or if you want to submit it electronically or complete it online and print it out, it is available in electronic format under the "Apply Online" section of their site.
