Final Few Days to Post Year 8 Form 470!
~ 41 Districts in PA Have NOT Filed, Listed Below ~

This is the final reminder to all PA applicants that these are the last few
days to post an E-rate Form 470 and still have time to file a Form 471
before the February 17 deadline. We still have 41 Districts that have yet
to file Form 470's (as of 1:30 p.m. on 1/13/05). The Districts and their
respective IUs are listed below so you can identify your neighbors that
haven't applied (please give them a call!)
The final deadline to post a Year 8 Form 470 is Tuesday, January 18. PLEASE
DON'T WAIT UNTIL JAN 18 TO FILE, BECAUSE THAT WILL ONLY GIVE YOU ONE
DAY (FEBRUARY 17) TO EVALUATE YOUR BID RESPONSES, SELECT VENDORS, SIGN
CONTRACTS (IF ANY), HAVE VENDORS SIGN CONTRACTS, AND COMPLETE AND SUBMIT
YOUR 471.
At this late date, you must file online at the SLD's website:
www.sl.universalservice.org (under Apply Online). I suggest using the
'Create Form 470' feature from the options and NOT the 470 Interview
Format. Please note: You can still file online even if you don't have a
PIN. EVERYONE can and should file online... the PIN is just to allow you to
certify your application online. If you don't have a PIN, hit submit, then
click on "Submit Paper Certification." Then print that page, sign, and mail
it to the SLD. Your
28 day clock begins when you hit the submit button.
There is still a chance that a district's name is listed but need not be,
because a district may have existing contracts for all E-rate eligible
services from previous years (and 470's was posted in previous years). If
this the case with your district, please contact me so I can take you off
this list.
