Many 486s OVERDUE for Yr 7, Waves 1-9

There are still 73 entities that have not filed 486s for Year 7 Funding
Requests that came in Waves 1-9. Because it is more than 120 days since the
funding commitment letters were mailed, this form is now OVERDUE. All 73
affected entities are listed below, along with the number of FRNs that need
to be listed on their 486, and the date that the 486 WAS DUE. Please note:
The FRNs listed are for priority 1 services only.
If your entity's name is listed below, please file the 486 TODAY. The SLD
is deducting 1/365th worth of funding for each day that the 486 is
postmarked after the due date, so every day counts.
A few notes:
* THERE IS NO EXTENSION REQUEST FOR THE 486. For each day that you file
late, your funding is reduced 1/365 until the funding has been depleted.
* If your school or library's name is listed below and you already mailed
your 486, the form likely is in the mail or waiting to be processed. If you
delegated this responsibility to someone else to complete or sign and mail,
please double check that it actually has been mailed. If you want to double
check that the SLD has received it, call the CSB at 888-203-8100 and they
can look it up. Sometimes there is a lag between the date that they
received it and when it finally gets processed (which is when I'm finally
able to see that they have it.)
* I encourage you to use the .pdf version of the online 486 available in the"Apply Online" section of the SLD website at www.sl.universalservice.org.
And if you have a PIN, you can certify that form online and not have to
mail-in the signature/certification page. If you don't have a PIN, you can
still submit your form online, but you must mail in the
certification/signature page, and it must be postmarked by the dates listed
below (hitting submit doesn't constitute a submitted form).
* You may include ALL of your funding requests on the same form. There is
no need to file a separate 486s for each FRN.
* If your service start date has been moved back to a date later than what
you listed on your 471, you may file a Form 500 to change the service start
date. The SLD will acknowledge this, then you will have 120 days after the
NEW service start date to submit your 486.
* If the service is not being used, you should file a Form 500 to cancel the
funding request.
