Year 8 (2005-2006) Funding Remains Uncollected for Internal Connections

The SLD's deadline to submit invoices is 120 days after the last day to receive service, and the last day to receive service for internal connections funding for Year 8 was September 30, 2006. Thus, the deadline to submit a BEAR or invoice to the SLD January 29, 2007 (Normally, the deadline is January 28, but this date falls on a Sunday, which makes the deadline the following Monday.) In PA, there is more than $542,586 that is yet uncollected and those affected entities are listed below, with details of each FRN attached to this message.
Please note that the list below and attached contains a listing of all applicants that have not submitted invoices for Year 8 priority 2 FRNs that have an invoice deadline on or just after 1/29/07. There are many other applicants that have until January 2008 to submit invoices because the SLD granted them automatic extensions due to the fact that they funded their applications after March 1, 2006.
Note, the attached spreadsheet shows whether a 486 has been filed yet for the FRN and also whether you will need to to submit a Form 500 to extend your contract expiration date through at least September 30, 2006. You need to complete this form(s) by January 29 as well. Both forms are available for downloading on the SLD's website at:

* If your school's name is listed below, AND you have received this service or equipment, AND you have paid for this service/equipment in full, you should complete a BEAR form, submit it to your vendor for their signature, then have it post marked to the SLD by January 29.
* If your school's name is listed below and you have received a discounted bill from your vendor, it is up to the vendor to invoice the SLD and you are not required to do anything but make certain the Form 486 was filed with the SLD. However, as a courtesy, you may wish to contact the affected vendor and let them know of this quickly approaching deadline.
* If your school's name is listed below and you no longer need this money, please complete a Form 500 and submit to the SLD to release funding so it can be available for other deserving applicants. This form also is located on the SLD's website, although you have to fill it out by hand. A type-in version is available on E-rate Central's website at:
