September 30 Deadline for Many P2 FRNs from FY 2005 and 2006

The last date to receive Internal Connections equipment or services is September 30 of any given funding year. Applicants that received late FCDLs or were approved for service substitutions, SPIN changes, or extension requests, were given extra time to receive such equipment/services and because so many FY 2005 FRNs were funded in January 2007, we have many FRNs in PA with a January 2007 deadline.
Attached is a spreadsheet that shows all FRNs in PA that have funding remaining for FY 2005 and FY 2006 and a presumed deadline of September 30, 2007. Although the SLD's publicly available data does not show the actual deadline for each FRN, I inserted a column with a presumed service delivery deadline based on the stated Invoice Deadline Date.
Anyone still wishing to receive equipment or services under these FRNs must submit a Service Delivery Deadline Extension Request (SDDE) to the SLD by September 30, 2007. There are no exceptions to this deadline however a SPIN change request or a Service Substitution Request submitted by September 30 also will provide an automatic Service Delivery Deadline Extension when it is approved. Please note:
* Some FRNs are listed that have had invoices paid by USAC, but with some funding remaining. I included those FRNs in case the applicant only received a portion of the equipment or services so far. If no further invoices are going to be submitted for this FRN, you are not required to submit a SDDE Request.
* If you submit a Service Delivery Deadline Extension Request and your Contract Expiration Date is on or prior to September 30, 2007, you also must submit a Form 500 to extend the expiration date of your contract.
* If you do not wish to use an FRN that is listed on the spreadsheets, please submit a Form 500 to cancel the FRN.
* The deadline for most of these FRNs to submit invoices is January 28, 2008. However if you apply for and receive a SDDE Request, you will automatically be given an extension to submit your invoices.
