E-rate Request for Embraq Customers

Embarq has contacted me to ask that all Pennsylvania E-rate applicants that have service with Embarq begin using the Embarq-Pennsylvania SPIN beginning with their FY 2008 (Year 11) E-rate applications.  I have listed below what I believe are the affected entities based on the SLD's FY 2007 databases.

The correct SPIN should be:  143001394.

-- If you used a different SPIN for your FY 2007 application, they are not asking you to go back and make any changes.

-- If you have already filed your Form 471 for FY 2008 using the 143019614 SPIN, please make a simple correction on your Receipt Acknowledgement Letter when you receive it, and fax it back to the SLD at 973-599-6526.
