Year 10 (2007-2008) Form 486s Due and Overdue for Many Entities! Message Posted February 20, 2008

The Form 486 is due 120 days from your FCDL date or the start of services (whichever is later), and so the Form 486 deadline varies depending on which wave you were funded. Below is a list of entities that have overdue or near overdue 486s. The relevant FRN numbers and actual Form 486 due date also are listed (urgent cases in red). If your name is on this list, please file your 486 ASAP. If your 486 is late, the SLD deducts 1/365 of funding for each day it's late. I will send an update each month reminding applicants whose 486s are due in the following month.
A few important notes:
* If you have not yet received your FCDL, you should not submit a 486 for those FRNs still pending SLD decision. When those applications are funded, those applicants will then have 120 days from the date of the FCDL to submit their 486.
* If your school or library's name is listed below and you already mailed your 486, the form likely is in the mail or waiting to be processed. If you delegated this responsibility to someone else to complete or sign and mail, please double check that it actually has been mailed.
* I encourage you to file online using the 486 available in the "Apply Online" section of the SLD website at www.sl.universalservice.org. If you have a PIN, you can certify that form online and not have to mail-in the signature/certification page. If you don't have a PIN, you can still submit your form online, but you must mail in the certification/signature page, and it must be postmarked by the dates listed below (hitting submit doesn't constitute a submitted form).
