May 17 is Final Day to Submit Item 21 Attachments

Tomorrow, May 17th, is the absolute final day to submit Item 21 Attachments for FY 2011 requests if you haven't done so already. If Item 21 attachments are not submitted for each FRN by May 17th, those FRNs are subject to denial. If this is found to be the case, PIA will notify the applicant with an "intent to deny" e-mail or fax, and applicants will have a certain number of days to prove that the attachments were submitted before the May 17 deadline.
At the end of April, USAC mailed and e-mailed reminders to the Form 471 contacts if the applicant had not submitted their Item 21 Attachments online. As a result, many applicants received unnecessary reminders because they submitted Attachments via fax, e-mail or regular mail.
If you submitted your Item 21 Attachments but received the "No Item 21 Attachment Reminder Letter" in the last two weeks, you may want to confirm USAC's receipt of your Item 21 attachments by submitting a query online using Submit a Question feature on the USAC website (http://www.slforms.universalservice.org/EMailResponse/EMail_Intro.aspx) or by calling the Client Service Bureau at 888-203-8100.
