Wednesday, May 9 is Final Day to Submit Item 21 Attachments

In mid-April, SLD sent out “Urgent Reminder” letters to applicants that either did not submit Item 21 attachments (or where the SLD could not positively identify which FRN the Item 21 attachment was related to), or did not submit the Form 471 certification. The absolute deadline for submitting the missing documents is May 9, 2012. If this deadline is missed, funding will be denied and applicants will be required to request a waiver from the FCC to have their E-rate funding requests reinstated.

If you submitted your Item 21 Attachments but received the "No Item 21 Attachment Reminder Letter" in the last two weeks, you should assume the Item 21 attachment was not received by USAC and resubmit it by Wednesday. In most cases, the 'urgent' letters were received by applicants that originally submitted their Item 21's via fax, e-mail or paper and did not clearly label the Item 21 attachment in the subject header, the file attachment name and on the attachment itself. FAX Item 21s 973-599-6511, or EMAIL them to attachments@sl.universalservice.org. Be sure you include your billed entity #, Form 471 #, Form Identifier and the FRN # on each Item 21 attachment and in the email or fax transmittal message.

If you need to certify a Form 471, you may do so online if the certifying person has an E-rate PIN, or sign and mail the paper certification to SLD (postmark must be by May 9th). To certify online or print a copy of the paper certification page, go to http://www.sl.universalservice.org/menu.asp and click on 'Certify Complete' under the Form 471 column. If you mail it, make sure you have a way to track delivery confirmation. If you would like to confirm your Form 471 has been certified, go to http://www.slforms.universalservice.org/Form471Expert/471StatusCheck.aspx and enter your Billed Entity #. Press search and then on the next screen scroll down and press the “continue” button. If your status is “complete,” this means SLD does not have a record of your certification. Do not be concerned if your status states “Certified – out of window.” After the May 9th deadline passes and all certifications have been processed, the SLD will change the status to “Certified-in window.”
