E-rate BEAR Deadline Not Extended

Although USAC offices were closed on Monday and Tuesday this week due to the storms, USAC has decided NOT to extend Monday's BEAR (Form 472) deadline. Therefore, if you did not have your paper BEAR postmarked or your online BEAR certified by your vendor by Monday (October 29), you must request an Invoice Deadline Extension. After USAC approves the request, you may then submit your BEAR to USAC.
A sample Invoice Deadline Extension is attached to this message. The USAC fax number to submit the request is not in-service, therefore you should submit the request using the online 'Submit a Question' feature on the USAC website. Here are the steps to follow:
1) Go to: http://www.slforms.universalservice.org/EMailResponse/emr_select.aspx
2) Under Topic Inquiry, select 'Invoice Deadline Extension Request' and 'Go'
3) Select 'Other' and then 'continue'
4) Complete the general information page and then 'continue'
5) On the next page you can either paste all of the information from your Invoice Deadline Extension Request.
