Year 6 (FY 2003-2004), Wave 12 E-rate Funding Committed

Congratulations to the following Pennsylvania school district that will be receiving a Year 6, Wave 12 E-rate Funding Commitment Letter, totaling $39,898.44.
A summary of the commitment is listed below, and detailed information is posted on the SLD web site under Funding Commitments.
If you find that your funding commitment letter does not arrive in a timely manner, please call the SLD ASAP to get a replacement (888-203-8100). All Year 6 correspondence is being sent in YELLOW.
If you do not agree with your funding commitment decision letter, you have 60 calendar days to appeal from the date listed at the top of your FCDL. Appeal instructions can be found on the SLD's website under the Reference Area.
Please let me know if you have questions concerning an appeal or would like me to review it before it's submitted.
A few very important notes:
YOUR FCDL IS NOT THE LAST STEP. YOU NOW MUST FILE THE FORM 486.
* The 486 tells the SLD 3 things:
1) that they are authorized to pay invoices (either BEAR or SPIFs)
2) which SLD-certified approver approved your technology plan
3) that you were CIPA compliant by start of services
This form must be filed within 120 of the service start date or the date on the funding commitment letter, whichever is later. And since most services begin on July 1, you have until October 29 to have this form filed with the SLD. If you are receiving internal connections funding, you may want to wait until after the product is delivered or installed to complete the 486.
The new version of the 486 is now available in paper form under the "Forms" section on the left side of the SLD website. It is not available electronically until the end of September.
