Form 470 Receipt Notification Letters - Where Are They?

Many of you have been asking why you didn't receive your Form 470 Receipt
Notification Letter for your Year 8 (2005-2006) applications. Well, the SLD
never mailed them. We're not sure why, and we've been told for several
months that they will eventually be mailed.
In the meantime, if you'd like to know your 470#, your Allowable Contract
Date, the date that your 470 was posted, or whether your 470 has been
certified yet, (basically all of the information that's contained on the 470
RNL), you can find the information easily on the SLD's website by following
the instructions below. The two most important pieces of information on the
posted Form 470 are these:
* Allowable Contract Date -- Be certain that you don't sign a contract or
post your Form 471 until this date or your application will be denied.
* Application Status -- Be certain this says, "CERTIFIED." If not, mail
another copy of your 470 certification page to the SLD. If you just mailed
it in the last 2 weeks, don't panic. But if you mailed it before that, you
should take the precaution and mail another copy. If you don't have a copy
of your certification page, follow the instructions below, but instead of
selecting "search posted" on # 3, click on "Certify Complete" to print a new
certification page.
1) Go to: www.sl.universalservice.org.
2) Click on "Apply Online" on the right toolbar.
3) Under the 470 column, click on "Search Posted."
4) Click "next" if you're looking for a 2005 (Year 8) Form 470.
5) Then either enter your entity number, your zip code, OR your 470#, and
then "next."
6) From there, click on the 470 that you would like to view. The
information you need will be listed at the top of the form.
