Year 8 Form 471 RALs Being Mailed

The SLD has begun mailing Year 8 Form 471 Receipt Acknowledgment Letters
(RALs) to applicants. A RAL is mailed for each Form 471 submitted and
contains much of the information contained in Block 5 for each FRN. As a
reminder, the color-code for Year 8 is blue.
The purpose of the RAL is to give the applicant a second chance to review
their funding request information and provide a mechanism for an applicant
to make corrections to the 471 before the application reaches PIA review.
REVIEW THIS DATA PROMPTLY. If the summarized data is accurate, no applicant
action is required. If there are changes to be made, or if you would like
to cancel an FRN, you can perform these tasks using the RAL but they request
that you do it within 3 weeks from the date on the RAL.
To make a correction, copy the first and last page of the RAL, cross out the
incorrect information and write in the correct information beside it (or
write CANCEL
next to the FRN). Then fax to the SLD per the instructions on the RAL.
A few notes:
1) Unfortunately, you cannot increase your request... you can only decrease
or cancel your request.
2) If you would like to make a change to Block 4 (discount calculations),
simply take a copy of your submitted application and make the correction to
that page. Then take that page along with the first page of the RAL, and
fax it back to the SLD.
3) Along those lines, if you would like to make corrections to any portion
of your submitted 471 (particularly Item 25d and 25f) that doesn't show on
the RAL, simply take a copy of your submitted application, make the
correction, attach it to the first page of the RAL, and fax it back to the
SLD.
4) If you have been contacted by PIA and you haven't received your RAL yet,
you should tell the PIA reviewer upfront that you were planning to make
corrections on your RAL. The reviewer will be able to make those changes
directly to your application without the need for you to proceed with the
RAL correction.
5) If you haven't yet done so, please go to the SLD website and print out a
copy of your completed application. To do so, follow these steps:
-- Go to: http://www.sl.universalservice.org/
-- Click on "Apply Online"
-- Under the 471 Column, click on "Display"
-- Enter your 471 Application Number and click "Original." If you have your
security code, you can enter this as well and it will show you more detailed
contact information. Without the security code, you will see everything in
the application except the personal information contained in the contact
fields.
