Year 7 (2004-2005) Funding Remains Uncollected for Internal Connections

The SLD's deadline to submit invoices is 120 days after the last day to receive service, and the last day to receive service for internal connections funding for Year 7 was September 30, 2005. Thus, the deadline to submit a BEAR or invoice to the SLD January 30, 2006 (next Monday. Normally, the deadline is January 28, but this date falls on a Saturday, which makes the deadline the following Monday.) In PA, there is more than $12 million that is yet uncollected and those affected entities are listed below.
Please note that some affected applicants are not listed below because they either received their FCDL after March 1 and thus were automatically granted an extra year to invoice the SLD or they have requested an invoice deadline extension.
* If your school's name is listed below, AND you have received this service or equipment, AND you have paid for this service/equipment in full, you should complete a BEAR form, submit it to your vendor for their signature, then have it post marked to the SLD by January 30.
Note, if there is a 'Y' next to your school's name, this means that a Form 486 has not been submitted to the SLD, and/or that you need to submit a Form 500 to extend your contract expiration date through at least September 30, 2006. You need to complete this form(s) by January 30 as well. Both forms are available for downloading on the SLD's website at:

* If your school's name is listed below and you have received a discounted bill from your vendor, it is up to the vendor to invoice the SLD and you are not required to do anything but make certain the Form 486 was filed with the SLD. However, as a courtesy, you may wish to contact the affected vendor and let them know of this quickly approaching deadline.
* If your school's name is listed below and you no longer need this money, please complete a Form 500 and submit to the SLD to release funding so it can be available for other deserving applicants. This form also is located on the SLD's website, although you have to fill it out by hand. A type-in version is available on E-rate Central's website at:
