More Year 8 (FY05) Form 486s Due Soon & Overdue for Many Entities!

For applicants that have had their Year 8 E-rate priority 1 funding requests approved, this is a another friendly reminder of the upcoming Form 486 deadline for applicants that haven't yet filed, and an urgent reminder for applicants whose 486 is not overdue. Because the waves are coming out weekly, and the 486 is due within 120 days of the funding commitment letter date or the start of services (whichever is later), the due dates of the 486 vary with each funding wave.
For example:
If you were funded on 10/12/2005 (wave 16), your 486 is due 2/8/2006.
If you were funded on 10/19/2005 (wave 17), your 486 is due 2/15/2006.
If you were funded on 10/26/2005 (wave 18), your 486 is due 2/22/2006.
If you were funded on 11/2/2005 (wave 19), your 486 is due 3/2/2006.
If you were funded on 11/9/2005 (wave 20), your 486 is due 3/9/2006.
Below is a list of entities that have 486s with quickly approaching deadlines or past deadlines, for which have not yet submitted their 486. If your name is on this list, please file your 486 ASAP. If your 486 is late, the SLD deducts 1/365 of funding for each day it's late. I will mail an update each month reminding applicants whose 486s are due in the following month.
A few important notes:
* If you have not yet received your FCDL, you should not submit a 486 for those FRNs still pending SLD decision. When those applications are funded, those applicants will then have 120 days from the date of the FCDL to submit their 486.
* If your school or library's name is listed below and you already mailed your 486, the form likely is in the mail or waiting to be processed. If you delegated this responsibility to someone else to complete or sign and mail, please double check that it actually has been mailed. If you have decided you don't need a particular FRN, file a Form 500 to permanently cancel the funding request (and get your name off these lists!)
* I encourage you to use the .pdf version of the online 486 available in the "Apply Online" section of the SLD website at www.sl.universalservice.org. If you have a PIN, you can certify that form online and not have to mail-in the signature/certification page. If you don't have a PIN, you can still submit your form online, but you must mail in the certification/signature page, and it must be postmarked by the dates listed below (hitting submit doesn't constitute a submitted form).
* You may include ALL of your funding requests on the same form. There is no need to file a separate 486s for each FRN.
