Which Entities Have Uncertified Year 9 Form 470's?

Any Form 470 cited on a Funding Year 9 Form 471 funding request must be certified prior to the close of the FY 2006 (Year 9) funding window on February 16, 2006. Below is a list of entities in PA that have not yet certified their 470s.
Last year, 3% of all E-rate denials (across the country) were due to untimely filed 470 certifications. Please just make sure that your entity is not on this list.
If your entity's name IS on the list, perhaps you mean for the 470 to die on the vine (and you won't ever be using it on a 471), or you mailed it and the SLD just hasn't entered it into their system yet (in which case you should call the SLD and check that they actually received it). If neither of these are the case, then go to the SLD website at http://www.sl.universalservice.org/menu.asp, and click on "Certify Complete" under the 470 column of the online application page (no need to enter the security code). You'll then have the choice of using your E-rate PIN to certify the form, or printing a paper copy and mailing it to the SLD. If you choose to mail it to the SLD, be sure it's dated by February 16, 2006 and that the SLD receives it.
