September 30 Delivery Deadline for Some FY 2007 FRNs

The last date to receive Internal Connections equipment or services is September 30 of the funding year (three months after the normal close to the funding year) and there are 15 entities that have received a funding commitment, but may not have yet received the equipment/services.
Attached is a spreadsheet that shows the affected 2007 FRNs, and the list of affected entities is listed below. Although the SLD's publicly available data does not show the actual deadline for each FRN, I inserted a column with a presumed service delivery deadline based on the stated Invoice Deadline Date.
-- If your entity's name is on the list and you have received the equipment, be sure your Form 486 has been filed, and then submit your invoice to USAC before the deadline.
-- If your entity's name is on the list and you have not received the equipment, submit a Service Delivery Deadline Extension Request (SDDE) to the SLD by September 30, 2008 to give yourself additional time to take receipt of the equipment. There are no exceptions to this deadline however a SPIN change request. Instructions for submitting a SDDE Request are available at: http://www.universalservice.org/sl/applicants/step11/service-deadlines-extension-requests.aspx. Be sure to include all of the requested information, including the required certifications.
Also note, if your Contract Expiration Date is prior to receipt of equipment/services, you'll also need to submit a Form 500 to extend the expiration date of your contract.
If your entity's name is on the list and you cannot use the E-rate funding, submit a Form 500 to cancel the FRN. A version that permits you to enter the data directly into the form is available at: http://www.e-ratecentral.com/formsRack/app/Form500.asp.
