September 30 Delivery Deadline for Some FY 2010 and 2011 FRNs

The last date to receive non-recurring services/equipment is September 30 of the funding year (three months after the normal close to the funding year), which means an important deadline is looming for several PA applicants that were funded but appear to not have invoiced USAC yet for the equipment (which could mean they have not yet received the equipment). Although September 30, 2012 is the deadline to receive FY 2011 non-recurring services/equipment, because much of the FY 2010 Priority 2 funding was awarded late and given automatic 1-year extensions, there are numerous FY 2010 applicants that also have the September 30, 2012 deadline.
In all, there are 45 entities in PA that have received a funding commitment, but may not have yet received the equipment/services. There is a strict deadline of September 30 to request a service delivery deadline extension if you will be receiving the equipment or services associated with the NRC after September 30, 2012. If your entity's name is on the list and you have not received all of the services/equipment associated with the NRC, submit a Service Delivery Deadline Extension Request (SDDE) to the SLD by September 30, 2012 to request an additional 12 months to take receipt of the equipment or have the network installed. There are no exceptions to this deadline. Instructions for submitting a SDDE Request are available at:
http://www.universalservice.org/sl/applicants/step11/service-deadlines-extension-requests.aspx. Be sure to include all of the requested information, including the required certifications. Also, you'll need to submit a Form 500 to extend the expiration date of your contract if the Contract Expiration Date listed on the attached spreadsheet is prior to receipt of equipment/services.
Attached is a detailed spreadsheet that shows the all possible affected 2010 and 2011 FRNs, and the list of affected entities also is listed below. This list does not include 2010 and 2011 FRNs that have been granted a service delivery deadline extension to September 30, 2013. Please note that it is very possible that the entities on the list have already received all of the equipment/services associated with these FRNs and just haven't invoiced USAC yet, in which case, they may ignore this message. If your entity is shown on this list and you will not be using this E-rate funding, please file a Form 500 to return the committed funds back to USAC.
