A summary of each commitment is listed below. All FY 2014 correspondence will be sent in BLUE.<br \/>\nA wave will be arriving every week until the FY 2014 funding is exhausted. This wave consists of requests for Priority 1 services (telecom and Internet). If you were not funded in this wave, don’t worry! There are many, many more waves coming, and your commitment will be in one of the upcoming waves.<br \/>\nThe next step for those entities funded in Waves 1-9 will be to file the Form 486 which tells the SLD it’s OK to pay invoices and that you are CIPA compliant. The Online Form 486 for FY 2014 is available and applicants listed below may file the form NOW as long as your recurring services will begin on or before July 31, 2014. The deadline to file the Form 486 is 120 days from the date of your funding commitment letter or October 28, 2014, whichever is later. If you’re unsure how to file the Form 486 or which boxes to check, please refer to the Form 486 Guide at https:\/\/e-ratepa.org\/Listserve-Archive\/2014\/05-16-14_2.html and the sample pages attached to that message.<br \/>\nIf you have any questions, please contact me at jtschell@comcast.net.
