Possible Form 500 Applicants<\/a><\/p>\n<p>~ Possible Affected Applicants Attached ~<\/p>\n<p>The last date to have FY 2016 Category 2, internal connections equipment delivered and installed is September 30, 2017.\u00a0 \u00a0If you have not yet received your FY 2016 internal connections (C2) equipment, or if you have received it but it is not yet installed, you must submit a Form 500 to request additional time., and that Form 500 must be submitted in EPC by Monday, October 2, 2017 (the normal deadline is September 30, but because this date falls on a Saturday, the deadline has been extended until Monday).<\/p>\n<p>Attached is a detailed spreadsheet that shows the all possible affected FY 2016 FRNs, and the list of affected entities also is listed below.\u00a0 The list was generated based on applicants who have not yet invoiced USAC for those FRNs.<\/p>\n<p>Notes:<\/p>\n<ul>\n<li>If you received your FY 2016 internal connections funding commitment letter (FCDL) on or after March 1, 2017, the rules automatically grant you a 12-month extension to purchase\/install the equipment.\u00a0 I\u2019ve included those entities in the spreadsheet, but made a note in the \u201cJulie Notes\u201d column to reflect that no Form 500 must be submitted.<\/li>\n<li>If your FY 2016 internal connections FCDL was received prior to March 1, 2017, and if you haven\u2019t purchased and installed the equipment yet, you must file the Form 500 by Monday to request the additional 12 months.<\/li>\n<li>If you are not going to use the FRN, you should file a Form 500 to cancel the FRN which then returns those dollars to your C2 budget for use in future years.<\/li>\n<\/ul>\n<p>If you have any questions, please don&#8217;t hesitate to contact me at <a href=\"mailto:jtschell@comcast.net\">jtschell@comcast.net<\/a>.<\/p>\n<p>&#8212;
