\n~ Be Careful to Check Correct Certifications! ~<\/p>\n
This is a friendly reminder that the Form 486 deadline is fast approaching for applicants that were funded in the first few waves of FY 2018.\u00a0 The 486 certifies receipt of services\/CIPA compliance, and is due within 120 days of the funding commitment letter date or the start of services (whichever is later).<\/p>\n
Attached is a list of all entities that have been funded for FY 2018 but who haven’t yet filed their Form 486, along with the Form 486 due date.\u00a0 If your Form 486 due date is in red, you must file this form ASAP in order to not lose E-rate funding.\u00a0 No BEARs or vendor SPIs can be submitted until the Form 486 has been certified\/approved.<\/p>\n
Deadline to Submit Form:<\/strong>\u00a0 120 days from funding commitment decision letter date, or the Service Start Date listed on the 486, whichever is later.\u00a0 For each 30 days that the 486 is overdue, USAC deducts 1 months\u2019 worth of E-rate funding.\u00a0 So don\u2019t delay \u2013 file the 486 as soon as you receive the FCDL!<\/p>\nAttached is a Form 486 Filing Guide that I encourage you to keep handy as you’re completing the 486.\u00a0 It explains what to click and not click to make the form even easier to navigate.\u00a0 Extreme Caution<\/strong>:\u00a0 When checking the certification boxes, don\u2019t get click happy!\u00a0 For all applicants except consortia, only check the 1st CIPA certification, and not the last two.\u00a0 If you check one of the last two, it unchecks the first certification and your application will be held for months until USAC reaches out to you to determine if you meant to report you weren\u2019t CIPA compliant.\u00a0 Here\u2019s what you SHOULD check.<\/p>\n\n
