This is a friendly reminder that the FY 2020 Form 486 is overdue or has not yet been filed for 24 PA applicants.\u00a0 The 486 certifies receipt of services\/CIPA compliance and is due within 120 days of the funding commitment letter date or the start of services (whichever is later).\u00a0 No BEARs or vendor SPIs can be submitted until the Form 486 has been certified\/approved.<\/p>\n

Attached is a list of all entities that have been funded for FY 2020 but who haven’t yet filed their Form 486, along with the Form 486 due date.\u00a0 I believe that some of these FRNs were created for the COVID second E-rate window.<\/p>\n

If you\u2019re unsure how to file the Form 486, please refer to the Form 486 Filing Guide at:\u00a0\u00a0https:\/\/e-ratepa.org\/wp-content\/uploads\/2020\/05\/Form-486-Filing-Guide-2020.pdf<\/a>.\u00a0 It explains what to click and not click to make the form even easier to navigate.<\/p>\n

Important note:<\/strong>\u00a0 If your Form 486 is marked OVERDUE, please file the Form 486, and then contact me immediately about filing an appeal with USAC to request that your Service Start Date be reset to July 1 (when you file the 486 late, USAC adjusts the Service Start Date to 120 days prior to the Form 486 filing date, thus taking away E-rate funding).<\/p>\n
