Next Steps?\u00a0 Review FCDL, File Form 486<\/p>\n

Congratulations to the following 9 PA schools and\u00a0libraries (30 separate FRNs), that were committed a total of $1,092,427.18 in Waves 14 and 15.\u00a0 These waves include\u00a0commitments for both Category 1 and Category 2 requests.\u00a0 A list of the funded applicants is below and a more detailed analysis of these funding waves is attached.\u00a0 If you are included on this list, your Form 471 contact and EPC Account Administrator (AA) would have received an e-mail with a .pdf of the FCDL(s) attached.\u00a0 Please review the FCDL carefully to make sure you agree with USAC\u2019s decision.\u00a0 Then file the Form 486, and contact your vendors to request discounted billing if you so desire.<\/p>\n

If you were NOT included in this wave,\u00a0do not worry!\u00a0\u00a0It just means that your application hasn’t yet been reviewed\/decided.\u00a0\u00a0Be watching for e-mail for notification that your PIA questions are available in EPC.\u00a0 As soon as the questions are answered, you’ll likely be in an upcoming funding wave.\u00a0 A funding wave will be released each week until all applications have been decided.<\/p>\n

A few notes:<\/strong><\/p>\n

— The next step is to file the Form 486 in EPC.\u00a0 If you file the 486 before July 31, check the Early Filing box on the form.\u00a0 Also, please be careful to check the accurate CIPA certifications on the final page (don\u2019t get click happy!).\u00a0 Please review the\u00a0Form 486 Filing Guide<\/a>\u00a0for assistance when filing this form.<\/p>\n

— If your FRN was modified or denied, the last page of your FCDL will explain why the funding request was modified.\u00a0 If you do not agree with the modification or the denial, please consider submitting an appeal to USAC within 60 day of the FCDL date. \u00a0If you\u2019re unsure whether you should appeal, please let me know and I can provide guidance.\u00a0 \u00a0An\u00a0FCDL User-Guide<\/a>\u00a0is available to help understand this form.<\/p>\n

— If you were funded for internal connections equipment, please review the\u00a0Category 2 Purchasing Guide<\/a>\u00a0to learn what information should be included on the PO and what your requirements are for asset inventories, etc.<\/p>\n

— The FCDLs are attached to the award e-mail notice and are in as .pdf format.\u00a0\u00a0 All EPC users in the organization\u2019s EPC account can download a copy on their EPC Landing Page under Notifications.<\/p>\n
