Form 498 – Have You Filed Yours Yet?

If you haven’t filed the Form 498 in EPC yet to provide USAC with your banking information, please do so soon.  Beginning immediately, all USAC reimbursements will be made only via direct deposit, so it’s important that you file the Form 498 with your school or library’s banking information so there’s plenty of time to have it approved before you file the BEAR for FY 2015.

If you filed the Form 498 and haven’t received the request from USAC to provide them with a copy of your voided or cancelled check, you may proceed directly to this website to upload this document: http://efileweb.usac.org/ContributorManagement/V1/BankValidation.  You will need to have your Form 498 Filer ID handy when you go to this link.  The Form 498 Filer ID can be found on the Form 498 that you filed.  It’s a 9-digit number that begins 443xxxxxx.  Note:  If you are in this situation (where you filed the 498 but have never been contacted by USAC), please let me know as we were told that everyone should have been contacted by last Wednesday and I’d like to confirm or refute that this has happened.)

To file the Form 498, log into EPC, click on the name of your organization, and then click on Related Actions.  If you don’t see the Form 498 among the list of forms to file, it means you are not listed as a School or Library Official under Manage User Permissions. If you are the Account Administrator for your entity, you can make this change yourself under Related Actions.  If you’re not the AA, contact the AA and ask that they log into EPC and change your permission setting.  Note:  a consultant cannot be designated a School or Library Official.

You’ll need these pieces of information in order to file the Form 498:

  • Name of General Financial Contact (must be an EPC user and must be a school or library official)
  • Name of Remittance Contact (can be anyone, regardless of whether they’re in EPC)
  • Name of Bank
  • School or Library’s Federal EIN Number
  • DUN and Bradstreet Number (DUNS) (the Guide explains where to quickly find this data)
  • Financial Institution Account Number for ACH
  • ACH Financial Institution Transit Number

Attached is a copy of the Form 498 Filing Guide that will help you through each page of the form.  It’s a very simple (short) form to file.
