September 30 Internal Connections Delivery Deadline for FY 2016 FRNs

The last date to have FY 2016 Category 2, internal connections equipment delivered and installed is September 30, 2017 which means an important deadline is just four weeks away for 224 PA applicants.   Below is a list of PA applicants that may not have received and/or installed their FY 2016 internal connections equipment yet (based on the fact that either no invoice has been submitted yet to USAC, or an invoice has been submitted but not for the entire amount of the FRN).  To obtain additional time to receive/install the equipment, applicants must submit a Form 500 to request a 12-month Service Delivery Deadline Extension (SDDE) and that Form 500 must be submitted online in EPC by September 30, 2017.  There are no exceptions to this deadline.

Attached is a detailed spreadsheet that shows the all possible affected FY 2016 FRNs, and the list of affected entities also is listed below. A few important notes:

• If no Form 486 has been filed yet, it is noted in red.
• If the vendor has not submitted their FY 2016 Form 473 SPAC Form with USAC, they must do so before any BEAR or vendor invoices can be submitted to USAC.
• If you need an additional 12 months to purchase or install the equipment, a request must be made on a Form 500  in EPC, and the form must be submitted on or before September 30, 2017 – no exceptions.
• If your name is on the list and you have received/installed the equipment and have already invoiced USAC), but will not be using the remaining funding (or won’t be using the FRN at all), you will want to file a Form 500 to return the remaining Category 2 funding to that school’s Category 2 budget so the money can be used in future years.  The Form 500 must be filed in EPC.  It takes several months for those forms to be processed, FYI.
• If you received the equipment, but have not yet installed it, the deadline for all equipment to be installed is also September 30, 2017.
