Form 498 – Adding UEI Number & Banking Info – Updated List

Good morning, E-Rate community.

After sending an email yesterday that included data pulled from USAC’s new Applicant 498 UEI Confirmation Tool, I’ve heard from a number of applicants who weren’t on the list.  I pulled the data again this morning, and it seems more complete.  You can access the updated list at:

The list represents applicants who have previously filed a Form 498 in EPC.  If you see “No” listed in Column “B” for your organization, you still need to add your SAM.gov UEI number to your existing Form 498.  This is required in order to receive reimbursement when filing Form 472/BEAR beginning in August 2026.For step-by-step instructions on updating/modifying the existing Form 498, or how to file a new Form 498 if there is no current approved form in EPC, I’ve provided a guide you can access by clicking on

If your organization has never filed a Form 498 in EPC, which is common if you’ve only used the SPI invoicing method (discounted billing), you will not be listed in the USAC data tool.  If your organization has filed a Form 498, but is still missing from the dataset, please let me know, and I’ll reach out to USAC to determine the issue.
