Districts That May Need to File 470′s for FY 2016

Although I said in a previous message that I wasn’t going to send a list of districts that needed to file 470′s, I changed my mind.  Below is a list of the 56 districts that had broadband or Internet funding requests from FY 2015 which were either MTM services or had contracts expiring on or before June 30, 2016 – AND – for which no 470 has been posted for FY 2016.  This list does not include entities that may have expiring or MTM contracts for voice services.

If you have a contracted broadband/internet service that contained voluntary extensions and you are choosing to exercise one of those extensions, please disregard this message.  Otherwise, based on the data you entered on your FY 2015 Form 471, you must file a FY 2016 Form 470 for these services if you want to include them on your FY 2016 Form 471.  I included the FY 2015 service provider name to give you an idea of which service/contract I’m seeing that is either MTM or expiring soon.  Don’t forget, you cannot extend a contract unless your original contract contained the contract extension options and those options are finite.  Meaning, it cannot be open ended or automatically renew each year.  It should say something like, 5-year term with 3, 1-year renewal options.  If it contains renewal options, then you don’t have to file a 470 during those “extension” years.

If your district is listed below, please file your 470 ASAP. 

A few reminders: 

1)  All 470′s must be filed in EPC.  If your district is not yet logged-into EPC, please call USAC at 888-203-8100 and they can identify who is the EPC Account Administrator for your entity.

2)  Upload RFPs into 470:   If issuing an RFP, it must be uploaded with your 470 in EPC at the time of filing.  Also, any addenda that are added after the 470 is filed also must be uploaded into EPC.   

3)  List disqualification reasons in State/Local Procurement Rules or in the RFP.  If you want to disqualify vendors, you must list those disqualification reasons in either your Narrative Section, the State/Local Procurement Rules Section, or in your RFP (if you have one).

4)  State and local procurement requirements?  Yes!  This is the space where you place instructions on bidders, such as the final date to submit bids (can’t be earlier than 28 days after posting the 470), where to submit bids, in what format, if references are required, if vendors must have SPIN numbers, etc.  Use this space wisely!
