September 30 Internal Connections Delivery Deadline for FY 2018 FRNs

Attached: FY 2018 Sept 30 Deadline for Int Conn FRNs that May Need More Time

The last date to have FY 2018 Category 2, internal connections equipment delivered and installed is September 30, 2019 which means an important deadline is just 10 days away.   Below is a list of PA applicants that may not have received and/or installed their FY 2018 internal connections equipment yet (based on the fact that no invoice has been submitted yet to USAC).  To obtain additional time to receive/install the equipment, applicants must submit a Form 500 to request a 12-month Service Delivery Deadline Extension (SDDE) and that Form 500 must be submitted online in EPC by September 30, 2019.  There are no exceptions to this deadline.

Attached is a detailed spreadsheet that shows all FY 2018 FRNs where no invoices (BEARs or SPIs) have been paid by USAC; the list of affected entities also is listed below. A few important notes:

• If no Form 486 has been filed yet, it is highlighted in yellow (Column I).
• If the vendor has not submitted their FY 2018 Form 473 SPAC Form with USAC, it is noted in red (Column H).  The Form 473 must be submitted by the vendor before any BEAR or vendor invoices can be submitted to USAC.
• If you need an additional 12 months to purchase or install the equipment, a request must be made on a Form 500 in EPC, and the form must be submitted on or before September 30, 2019 – no exceptions.
• If your name is on the list and will not be using the funding, you will want to file a Form 500 to return the Category 2 commitment.  The Form 500 must be filed in EPC.
• If you received the equipment, but have not yet installed it, the deadline for all equipment to be installed is also September 30, 2019.
• If you purchased the equipment but are on the list because you haven’t yet invoiced USAC, remember that the deadline to submit the BEAR or SPI to USAC is January 28, 2020.  If you request an extension, your new invoicing deadline will be February 28, 2020.
