This is a friendly reminder that the FY 2019 Form 486 is overdue or has a fast approaching deadline for 30 PA applicants.  The 486 certifies receipt of services/CIPA compliance, and is due within 120 days of the funding commitment letter date or the start of services (whichever is later).

Attached is a list of all entities that have been funded for FY 2019 but who haven’t yet filed their Form 486, along with the Form 486 due date.  If your Form 486 due date is in red, you must file this form ASAP in order to not lose E-rate funding.  No BEARs or vendor SPIs can be submitted until the Form 486 has been certified/approved.  (Note, there are a few entities that may have filed their 486s but who haven’t had them approved yet by USAC.  Those FRNs are still on the list.)

Deadline to Submit Form:  120 days from funding commitment decision letter date, or the Service Start Date listed on the 486, whichever is later.  For each 30 days that the 486 is overdue, USAC deducts 1 months’ worth of E-rate funding.

Filing Late?  Complete these 2 steps:  1) File the 486 ASAP.  USAC will then send you a 486 approval letter, but will adjust your service start date to 120 before the 486 filing date which will thus result in a reduction of funding.  2) Contact me and I will help you file an appeal with USAC to have your funding restored with a July 1 service start date.

Filing Guide:  A Form 486 Filing Guide is available at:  http://e-ratepa.org/wp-content/uploads/2019/05/Form-486-Filing-Guide-FY-2019.pdf that I encourage you to keep handy as you’re completing the 486.  It explains what to click and not click to make the form even easier to navigate.  Extreme Caution:  When checking the certification boxes, don’t get click happy!  For all applicants except consortia, only check the 1st CIPA certification, and not the last two.  If you check one of the last two, it unchecks the first certification and your application will be held for months until USAC reaches out to you to determine if you meant to report you weren’t CIPA compliant.  Here’s what you SHOULD check.

CIPA Certifications for Consortia:  Before Consortia can file their 486, they must first collect a signed Form 479 (each year) from each consortia member.  After all 479s have been collected, the consortia lead may file the Form 479 and check the following CIPA certifications.
